Application Forms

Forms for application to the graduate school program are available here. The following links will provide you with a printable version of each form. In order to view these forms, you will need to have Adobe Acrobat Reader installed on your computer. If you do not, you can download a free version here.

Registration form: Complete and mail the registration form by June 1, 2013.

Application form: Complete an application and submit it with a $25 nonrefundable application fee to the director of graduate education at Dordt College. Applications for admission for the summer 2013 are due by June 1, 2013.

Housing form: If you require on-campus housing, complete and submit by May 31, 2013.

Reference form: Provide a reference form to someone who is qualified to judge your professional abilities and potential as a graduate student. Teachers should submit one reference from their principal or supervising teacher.

Transcript request: Request official transcripts from your undergraduate degree-granting institution and any institution where you have taken courses for graduate credit. These should be sent directly to the director of graduate education at Dordt College.
